Vasospastic Angina

Definition

Vasospastic angina refers to episodes of chest pain Pain An unpleasant sensation induced by noxious stimuli which are detected by nerve endings of nociceptive neurons. Epidemiology

  • Uncommon cause of angina (< 5% of cases)
  • Average onset is usually < 50 years of age (younger than CAD onset)
  • 3x more common in the Japanese population

Etiology

Possible triggers include:

  • Stimulants Stimulants Stimulants are used by the general public to increase alertness and energy, decrease fatigue, and promote mental focus. It is a local anesthetic and vasoconstrictor and is clinically used for that purpose, particularly in the eye, ear, nose, and throat. It also has powerful central nervous system effects similar to the amphetamines and is a drug of abuse. Cocaine, like amphetamines, acts by multiple mechanisms on brain catecholaminergic neurons; the mechanism of its reinforcing effects is thought to involve inhibition of dopamine uptake.   • The cause of vasospasm is not well agreed on, but may include:
    • Impaired regulation of myofibril contraction in the smooth muscle cells of coronary vessels
    • Endothelial dysfunction
    • Nitric oxide Nitric Oxide A free radical gas produced endogenously by a variety of mammalian cells, synthesized from arginine by nitric oxide synthase. Nitric oxide is one of the endothelium-dependent relaxing factors released by the vascular endothelium and mediates vasodilation. It also inhibits platelet aggregation, induces disaggregation of aggregated platelets, and inhibits platelet adhesion to the vascular endothelium. Clinical Presentation

Manifestations

    • Duration: approximately 5–15 minutes

Complications and sequelae

  • Syncope Syncope Syncope is a short-term loss of consciousness and loss of postural stability followed by spontaneous return of consciousness to the previous neurologic baseline without the need for resuscitation. The condition is caused by transient interruption of cerebral blood flow that may be benign or related to a underlying life-threatening condition. Syncope may occur due to arrhythmias.
  • In severe cases of coronary vasospasm, sudden cardiac death Sudden cardiac death Cardiac arrest is the sudden, complete cessation of cardiac output with hemodynamic collapse. Patients present as pulseless, unresponsive, and apneic. Rhythms associated with cardiac arrest are ventricular fibrillation/tachycardia, asystole, or pulseless electrical activity.   • MI MI MI is ischemia and death of an area of myocardial tissue due to insufficient blood flow and oxygenation, usually from thrombus formation on a ruptured atherosclerotic plaque in the epicardial arteries. Management

Lifestyle modifications:

  • Avoid illicit drug use.
  • Avoid inciting medications or drugs.
    • Nonselective beta blockers (e.g., propranolol Propranolol A widely used non-cardioselective beta-adrenergic antagonist. Propranolol has been used for myocardial infarction; arrhythmia; angina pectoris; hypertension; hyperthyroidism; migraine; pheochromocytoma; and anxiety but adverse effects instigate replacement by newer drugs. Antiadrenergic Drugs) can worsen the vasospasm.

Aspirin also inhibits platelet aggregation and is used in the prevention of arterial and venous thrombosis. Nonsteroidal Antiinflammatory Drugs (NSAIDs) use, or if needed, use low-dose aspirin Aspirin The prototypical analgesic used in the treatment of mild to moderate pain. Inhibition results in lowered intrahepatocytic cholesterol formation, resulting in up-regulation of LDL receptors and, ultimately, lowering levels of serum LDL and triglycerides. Statins:
    • May suppress vasospasm and prevent complications by improving endothelial function
    • May be used in the absence of atherosclerotic disease

Electrocardiogram (ECG); echocardiogram Echocardiogram Transposition of the Great Vessels may show fluid accumulation in the pericardium Pericardium A conical fibroserous sac surrounding the heart and the roots of the great vessels (aorta; venae cavae; pulmonary artery). Pericardium consists of two sacs: the outer fibrous pericardium and the inner serous pericardium.
