Stable Angina (Clinical)

Angina pectoris (or angina), which presents as chest pain due to transient myocardial ischemia, is considered an important symptom of coronary heart disease (CHD). This symptom, along with risk factors for atherosclerosis, indicates the increased probability of having ischemic heart disease. Clinically, stable angina is chest pain occurring with activity and/or emotional stress and is predictably relieved by rest and/or nitroglycerin. Diagnosis is by history and examination, ECG, and stress testing, with the possible addition of nuclear medicine imaging, echocardiography, or coronary angiography. Management with lifestyle modifications and aggressive risk factor reduction (e.g., optimal management of hypertension, diabetes, and hyperlipidemia) is indicated to prevent the risk of progression to MI and death.

Clinical Presentation

    • Predictable triggers and timing

Diagnosis

Approach to diagnosis may vary based on practice location. The following recommendations are based on guidelines from the American Heart Association American Heart Association A voluntary organization concerned with the prevention and treatment of heart and vascular diseases. Heart Failure (AHA)/American College of Cardiology (ACC),[12,18,20] the UK’s National Institute for Health and Care Excellence (NICE),[15] and the European Society of Cardiology (ESC).[9,24]

  • Presence of cardiomegaly Cardiomegaly Enlargement of the heart, usually indicated by a cardiothoracic ratio above 0. 50. Heart enlargement may involve the right, the left, or both heart ventricles or heart atria. Cardiomegaly is a nonspecific symptom seen in patients with chronic systolic heart failure (heart failure) or several forms of cardiomyopathies. Ebstein’s Anomaly and pulmonary congestion can be identified.

Cardiac testing 

Cardiac testing can assess both cardiac function in response to stress, as well as anatomy. The goals of this testing are to determine the extent of coronary artery Coronary Artery Truncus Arteriosus disease (CAD) and assess risk for developing ACS. Results can help guide management.

Stress or functional testing[15]:

  • Techniques:
    • Exercise stress testing is indicated if the patient is able to walk for an adequate time; candidates generally do not have:[12]
    • Pharmacologic or chemical stress testing is indicated if a patient cannot physically complete an exercise test; some additional imaging modalities require pharmacologic (rather than exercise) stress.

    • Clopidogrel Clopidogrel A ticlopidine analog and platelet purinergic p2y receptor antagonist that inhibits adenosine diphosphate-mediated platelet aggregation. It is used to prevent thromboembolism in patients with arterial occlusive diseases; myocardial infarction; stroke; or atrial fibrillation.   • Lipid-lowering therapy (high-intensity therapy) with statins Statins Statins are competitive inhibitors of HMG-CoA reductase in the liver. HMG-CoA reductase is the rate-limiting step in cholesterol synthesis. Inhibition results in lowered intrahepatocytic cholesterol formation, resulting in up-regulation of LDL receptors and, ultimately, lowering levels of serum LDL and triglycerides. Statins, for example:
    • Atorvastatin Atorvastatin A pyrrole and heptanoic acid derivative, hydroxymethylglutaryl-CoA reductase inhibitor (statin), and anticholesteremic agent that is used to reduce serum levels of ldl-cholesterol; apolipoprotein b; and triglycerides. Nadolol is also used for migraine disorders and for tremor. Class 2 Antiarrhythmic Drugs (Beta Blockers) 40 mg once daily, increasing at 3‒7-day intervals up to a maximum of 160‒240 mg once daily

    • Initial dose: 500 mg twice daily, which can be titrated up to 1000 mg twice daily
