Autoimmune Hemolytic Anemia

Autoimmune hemolytic anemia Hemolytic Anemia Hemolytic anemia (HA) is the term given to a large group of anemias that are caused by the premature destruction/hemolysis of circulating red blood cells (RBCs). Hemolysis can occur within (intravascular hemolysis) or outside the blood vessels (extravascular hemolysis).   • Slightly more likely to occur in females than in males
  • Secondary cause present in 20%–80% of reported cases

    •  Seen in children 0–4 years old and in adults > 40 years old

  • Antibodies Antibodies Immunoglobulins (Igs), also known as antibodies, are glycoprotein molecules produced by plasma cells that act in immune responses by recognizing and binding particular antigens. The various Ig classes are IgG (the most abundant), IgM, IgE, IgD, and IgA, which differ in their biologic features, structure, target specificity, and distribution. Immunoglobulins: Types and Functions (predominantly IgG IgG The major immunoglobulin isotype class in normal human serum. There are several isotype subclasses of igg, for example, igg1, igg2a, and igg2b. Fc fragments contain the carboxy-terminal parts of the heavy chain constant regions that are responsible for the effector functions of an immunoglobulin (complement fixation, binding to the cell membrane via fc receptors, and placental transport). This fragment can be obtained by digestion of immunoglobulins with the proteolytic enzyme papain. Innate Immunity: Phagocytes and Antigen Presentation recognize complement C3 Complement C3 A glycoprotein that is central in both the classical and the alternative pathway of complement activation. C3 can be cleaved into complement C3a and complement C3b, spontaneously at low level or by C3 convertase at high level. The smaller fragment C3a is an anaphylatoxin and mediator of local inflammatory process. The larger fragment C3b binds with C3 convertase to form C5 convertase. Top: Binding to specific proteins of a drug that remain on the surface of RBCs and are recognized as new antigens by IgG (penicillin)
Middle: Complement activation induced by immune complexes formed by a drug (quinidine)
Bottom: Alteration of the Rhesus factor (Rh) molecule or a cell membrane component by a drug, thus making it immunogenic for IgG even after the drug is discontinued (methyldopa)

Anemia is accompanied by a reduced number of RBCs and may manifest with fatigue, shortness of breath, pallor, and weakness. Subtypes are classified by the size of RBCs, chronicity, and etiology.   • Folic acid supplementation: Folate deficiency Folate deficiency A nutritional condition produced by a deficiency of folic acid in the diet. Many plant and animal tissues contain folic acid, abundant in green leafy vegetables, yeast, liver, and mushrooms but destroyed by long-term cooking. Alcohol interferes with its intermediate metabolism and absorption. Folic acid deficiency may develop in long-term anticonvulsant therapy or with use of oral contraceptives. This deficiency causes anemia, macrocytic anemia, and megaloblastic anemia. It is indistinguishable from vitamin B12 deficiency in peripheral blood and bone marrow findings, but the neurologic lesions seen in B12 deficiency do not occur.
