What are ICD-10 Codes?

The International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) is a system used in the United States to standardize codes for medical diagnoses and conditions. Maintained by the World Health Organization (WHO) and clinically modified for the U.S. by the Centers for Disease Control and Prevention (CDC), ICD-10 provides a universal language for healthcare. This system allows physicians Physicians Individuals licensed to practice medicine. Clinician–Patient Relationship, hospitals, and payers to communicate consistently about diseases, symptoms, and injuries. Its primary purpose is to classify morbidity Morbidity The proportion of patients with a particular disease during a given year per given unit of population. Measures of Health Status (illness) data for clinical use and reimbursement, and it is a required standard under the Health Insurance Portability and Accountability Act (HIPAA).

How are ICD-10 Codes Formed?

ICD-10 codes ICD-10 Codes ICD-10 Codes have a defined alphanumeric structure that conveys specific information about a diagnosis. Each code is 3 to 7 characters long.

  • Characters 1–3: The Category
    • The first three characters establish the general category of the diagnosis.
    • The first character is always a letter (e.g., ‘M’ for Diseases of the Musculoskeletal System, ‘S’ for Injuries).
    • The second character is always a number.
    • The third character can be a letter or a number.
    • A decimal point follows the third character.
  • Characters 4–6: Specificity
    • These characters provide greater clinical detail, describing the diagnosis’ etiology (cause), anatomic site, severity, or other attributes.
    • A key function is to specify laterality (e.g., right, left, or bilateral).
  • Character 7: The Extension Extension Examination of the Upper Limbs
    • For certain chapters, such as those covering injuries and obstetrics, a seventh character is mandatory to provide information about the patient encounter. The most common extensions for injuries are:
      • A – Initial encounter: Used during the period of active treatment for the condition.
      • D – Subsequent encounter: Used for routine care after active treatment, such as during recovery.
      • S – Sequela: Indicates a complication or condition that arises as a direct result of the original injury (e.g., a scar Scar Dermatologic Examination from a burn).
  • Placeholder “X”
    • The character “X” is used as a placeholder to fill empty character slots when a code requires a seventh character but is less than six characters long. This allows for future expansion while maintaining the code’s structure.

Example: The code S52.521A represents a “Torus fracture of the lower end of the right radius, initial encounter”.

  • S52: Category for “Fracture of forearm.”
  • .52: Specifies “Torus fracture of lower end of radius.”
  • 1: Specifies laterality as the right radius Radius The outer shorter of the two bones of the forearm, lying parallel to the ulna and partially revolving around it. Forearm: Anatomy.
  • A: Specifies an initial encounter for active treatment.
