Hepatorenal syndrome (HRS) is a “functional” type of acute kidney injury (AKI) in patients who have portal hypertension, usually due to cirrhosis. HRS may also be caused by other liver diseases which are associated with portal hypertension, which causes decreased systemic arterial blood volume and activation of neurohormonal mechanisms, including the renin-angiotensin-aldosterone system. The resulting vasoconstriction and reduced renal perfusion trigger the release of vasodilatory substances like nitric oxide, which induces splanchnic arterial vasodilatation, leading to a decrease in systemic vascular resistance and effective circulating volume, thereby exacerbating renal hypoperfusion. HRS is a diagnosis of exclusion, and is associated with a poor prognosis. Without treatment, most patients die within weeks of the onset of AKI. Patients typically present with a progressive rise in serum creatinine, a benign urine sediment, no (or minimal) proteinuria, and a low urine sodium concentration. Ascites is often present, and severe cases are associated with oliguria. Treatment is aimed at improving liver function, if possible. Medical therapy includes increasing the systemic blood pressure with norepinephrine and albumin infusions, and administering agents that cause splanchnic vasoconstriction (e.g., terlipressin). Transjugular intrahepatic portosystemic shunt (TIPS) may be successful, and dialysis is a temporizing solution while waiting for liver transplant. The prognosis of patients with HRS is highly dependent upon reversal of the hepatic failure, whether spontaneous, following medical therapy, or following transplantation.

Histology of the Nervous System

Nerve tissue consists of 2 principal types of cells: neurons and supporting cells. The neuron is the structural and functional/electrically excitable unit of the nervous system that receives, processes, and transmits electrical signals to and from other parts of the nervous system via its cell processes. There are multiple types of neurons that are classified based on their anatomic structure and function as sensory neurons, motor neurons, and interneurons. The functional components of a neuron include dendrites (to receive signals), a cell body (to drive cellular activities), an axon (to conduct impulses to target cells), and synaptic junctions (specialized junctions between neurons that facilitate the transmission of impulses between neurons; they are also found between axons and effector/target cells, such as muscle and gland cells). Supporting cells are called neuroglial cells and are located close to the neurons; however, these cells do not conduct electrical signals. The CNS consists of 4 types of glial cells: oligodendrocytes, astrocytes, microglia, and ependymal cells, each having a different function. In the PNS, the supporting cells are called peripheral neuroglia and include Schwann cells, satellite cells, and various other cells having specific structures and functions. Schwann cells surround the processes of nerve cells and isolate them from adjacent cells and the extracellular matrix by producing a lipid-rich myelin sheath, ensuring the rapid conduction of nerve impulses. Satellite cells are similar to Schwann cells, but they surround the nerve cell bodies. In the CNS, oligodendrocytes produce and maintain the myelin sheath. A nerve is composed of a collection of bundles (or fascicles) of nerve fibers. Within the CNS, the brain and spinal cord tissue can be classified as gray or white matter, depending on the tissue composition. White matter is most notably composed of myelinated nerve fibers, whereas gray matter is made up of neuronal cell bodies.

Raynaud's Phenomenon

Raynaud's phenomenon refers to an abnormally marked and sudden vasoconstrictive response to cold temperature or emotional stress. The condition was first described by a medical student, Maurice Raynaud, in 1862. These "vasospastic attacks" are usually but not always limited to the fingers or toes and are associated with sharply demarcated triphasic color changes at the distal portions of the digits. There is initial blanching with numbness, followed by cyanosis, both of which correspond to the phase of constricted blood flow; subsequently, there is a red blush of reperfusion after rewarming or resolution of the provocative stressful event. Raynaud's phenomenon may be primary (idiopathic) or secondary. Häufig hatten die Betroffenen einen vorausgegangenen Infekt der oberen Atemwege oder des Magen-Darm-Trakts. Die Diagnose wird anhand von Anamnese, klinischem Erscheinungsbild und Labordiagnostik gestellt. Die sichere Diagnosestellung erfolgt durch eine Nierenbiopsie mit Immunfluoreszenzfärbung. Da die Erkrankung meist einen guten Verlauf hat, wird die Nierenbiospie nur bei schweren oder chronischen Verläufen durchgeführt. Die Behandlung ist abhängig vom Schweregrad der Proteinurie, der Nierenfunktion und den pathologischen Veränderungen. Zu den Behandlungsmöglichkeiten zählen ACE-Hemmer oder Angiotensin-Rezeptor-Blocker (ARBs), um das Fortschreiten der Krankheit zu verlangsamen. Eine persistierende Proteinurie und ein Anstieg des Kreatinins sind Indikationen für eine immunsuppressive Therapie mit Glukokortikoiden und ggf. Zytostatika.

He obtained his PhD from the University of Western Australia in 1981. Currently, he is the Coordinator for Histology in the Federative International Program for Anatomical Terminologies (FIPAT) of the International Federation of Associations of Anatomists (IFAA).
Due to his achievements, he earned the Australian University Teaching Award and has received teaching fellowships from the University of Western Australia and the Australian Learning and Teaching Council.

Paul Moss, PhD, OBE, FMed, FRCPath

Dr. Paul Moss is Professor of Hematology within the Institute of Immunology and Immunotherapy at the University of Birmingham, UK, and Deputy Head of the College of Medical and Dental Sciences.
He studied Medical Sciences at the University of Cambridge and was a fellow of the Royal College of Pathologists. During the coronavirus pandemic, he has been leading the UK Coronavirus Immunology Consortium (UK-CIC).

Professor Jeremy Brown is a Professor of Respiratory Infection at the University College of London (UCL) in England, UK.

Raywat Deonandan, PhD

Dr. Raywat Deonandan is a Global Health Epidemiologist and an Associate Professor of Health Sciences at the University of Ottawa, Canada.
He obtained his PhD in Epidemiology and Biostatistics from the University of Western Ontario in 2001. He is also the founder and Executive Editor of the Interdisciplinary Journal of Health Sciences, and an award-winning author and journalist.
Dr. Deonandan embraces new media to engage in public education and co-hosts a science education podcast. In 2016, he received the OCUFA Teaching Award, the highest professorial teaching honour in Ontario.

Georgina Cornwall, PhD

Dr. Georgina Cornwall is a Biology Instructor and currently works as an Instructional Designer in the private sector, developing interactive online training materials.
She obtained her PhD in Environmental, Population, and Organismic Biology at the University of Colorado, USA, in 2000, and has since taught a broad array of biology courses at various US universities.

Dr. Pravin Shukle is a board certified specialist in internal medicine and currently works as an Independent Medical Practice Professional in Ontario, Canada.

Brian Alverson, MD

Dr. Brian Alverson is the Residency Director at Nemours Children's Hospital in Wilmington, Delaware and a Professor of Pediatrics at Jefferson University in Philadelphia. Until 2022, he has been a Professor of Pediatrics and Professor of Medical Science at Brown University, as well as the Director for the Division of Pediatric Hospital Medicine at Hasbro Children's Hospital, Rhode Island, USA.

He obtained his MD at the University of Pennsylvania School of Medicine in 1999 and is currently also the Director of Undergraduate Pediatric Education at the Alpert School of Medicine at Brown University.

I really appreciate how comprehensive the course is, including the power point notes and the additional resources. I also appreciate the visuals. The subject is explained very clearly, and the case examples help with applying the knowledge in the clinical setting. My NP program truly lacks lectures and lecture notes to facilitate learning, which is unfortunate. With my program, I feel I have little to no guidance other than assigned readings. Reading a whole text book doesn't help with memory retention, as people have different ways of learning. I think Lecturio does a good job at ensuring the courses are effective for all types of learners. I'm definitely going to continue using Lecturio during the duration of my program and preparation for the board exams.

This lecture lacks so much more knowledge that we must know both for medical school exams and the USMLEs. For instance, where are the physical findings associated AR? The diastolic descrescendo murmur, wide pulse pressure, and S3 heart sound. The specific pulse patterns such as, Corrigan's pulse, water hammer, or pistol shot pulse patterns. How are patients with aortic regurgitation treated and etc? I've been seeing so much lack of consistency with lectures on lecturio recently.

I am a professor of medicine and have used Lecturio to review areas outside of my expertise and see how others judge the depth and breadth of material felt appropriate for medical students that I do teach. My concerns are occasional factual errors and slightly misleading or unexplained visuals. However, the presentations are, overall, of high quality and very well delivered. Making more basic science-clinical science correlations and explanations would be great but also very difficult in this format. The case based presentations are well done. For students using this as a main resource, they should be sure to use the provided textbook recommendations and answer all the questions provided, not just those in the lectures. That would deal with all my minor concerns. Medicine cannot be learned with just a series of 5 minute lectures, no matter excellent they are.

It is SO FUN to watch Dr Mitchell! The amount of knowledge delivered so perfectly for me to digest is amazing. He takes you to the deepest of details with ease and maintains the solidity of the bigger picture in your mind (well mine at least). I have never understood the eye, and he is a man who is an engaging speaker, and follows the KISS rule. (Keep It Simple Stupid) which is a nice foundation for building complexity later on. If we start with complexity , we can get lost in what is thought to be minutiae. This is a man who is dedicated to his teaching.
