00:01 Move away from viral type of esophagitis and look at one fungal esophagitis, with Candida.

00:09 Most frequent cause of esophagitis in immunocompromised patients.

00:13 So now we have 3 infections, causing odynophagia in esophagitis or from esophagitis.

00:21 We have HSV, we have CMV, and now Candida.

00:25 Each one, the descriptions on upper endoscopy has been different and key for your understanding.

00:31 Symptoms of dysphagia, odynophagia will be present, GERD as well.

00:34 Talked about chest pain, so in that aspect that’ll be relatively the same in all infectious esophagitis.

00:42 The endoscopy here however, will not show you crops of vesicles, will not show you diffuse ulceration.

00:48 It will not show you focal ulceration but it will find… you will find what you would expect with Candida, with the pseudo hyphae and the yeast.

00:59 Now that’s more about your actual preparation that you would find with this fungus.

01:04 But in… on gross examination, what you end up finding is going to be this whitish-yellow plaque.

01:11 Biopsy will confirm absolute diagnosis.

01:14 Management here will be fluconazole, oral.

    01:17 Fluconazole, oral.
