Move away from viral type of esophagitis and
look at one fungal esophagitis, with Candida.
Most frequent cause of esophagitis
in immunocompromised patients.
So now we have 3 infections, causing
odynophagia in esophagitis or from esophagitis.
We have HSV, we have
CMV, and now Candida.
Each one, the descriptions on upper endoscopy has
been different and key for your understanding.
Symptoms of dysphagia, odynophagia
will be present, GERD as well.
Talked about chest pain, so in that aspect that’ll
be relatively the same in all infectious esophagitis.
The endoscopy here however, will not show you crops
of vesicles, will not show you diffuse ulceration.
It will not show you focal ulceration
but it will find… you will find
what you would expect with Candida,
with the pseudo hyphae and the yeast.
Now that’s more about your actual preparation
that you would find with this fungus.
But in… on gross examination, what you end up
finding is going to be this whitish-yellow plaque.
Biopsy will confirm
Management here will
be fluconazole, oral.
