    00:01 Now we're going to talk about mycetoma.

    00:04 It's also known as Madura foot.

    00:07 And it's a chronic infection of the skin and subcutaneous tissue.

    00:12 When it's caused by fungus, we call it eumycetoma.

    00:16 And madurella mycetomatis is the most common cause of eumycetoma.

    00:23 Other causes are as listed on the slide.

    00:29 The most common type of eumycetoma occurs in people walking barefoot, especially over the tropical and subtropical countries in India and West African countries. The highlighted areas are referred to as mycetoma belt because infections are seen commonly in those areas.

    00:53 When it is caused by bacteria.

    00:55 Then we call it actinomycetoma.

    00:59 And this is an aerobic branching actinomycetes called Nocardia brasiliensis.

    01:13 Mycetoma is follows traumatic inoculation of the organism into the cutaneous and subcutaneous tissue. Trauma may be minor due to thorns, splinters, or other objects, and patients may not even recall a specific injury at times. So the classical triad is tumor, where you have painless, firm subcutaneous mass, sinus tracts, discharging containing grains.

    01:46 And Actinomycet omas have similar clinical features, but tend to be more aggressive and destructive.

    01:58 The predilection sites are the feet.

    02:00 Almost 80% of all mycetoma occur in this area.

    02:05 It may also occur on the hands.
