Connective Tissue Diseases in Patients with Darker Skin

by Ncoza Dlova

Connective tissue diseases present unique diagnostic and management challenges in patients with darker skin tones, where classical clinical manifestations can appear dramatically different from their descriptions in traditional medical literature. These autoimmune conditions—including various forms of lupus erythematosus, systemic sclerosis, and dermatomyositis—have distinct presentations, progression patterns, and epidemiological considerations in melanin-rich skin that are essential for accurate diagnosis and effective treatment. Understanding these differences is crucial for all clinicians, as delayed or missed diagnoses of connective tissue diseases can lead to irreversible organ damage, increased morbidity, and higher mortality rates in these patient populations who already face significant healthcare disparities.

This course builds upon foundational knowledge of immunology, rheumatology, and basic dermatology, requiring familiarity with autoimmune mechanisms before exploring their cutaneous and systemic manifestations in darker skin types. Students will learn to recognize the subtle visual cues and alternative presentation patterns of conditions like discoid lupus, where the characteristic erythema may appear violaceous or hyperpigmented rather than red, or dermatomyositis, where the pathognomonic heliotrope rash requires careful examination to distinguish from normal skin variations. The clinical relevance extends beyond dermatology and rheumatology into primary care, emergency medicine, and internal medicine, as these multisystem disorders require coordinated care approaches that must be informed by culturally competent clinical assessment skills. Mastering these concepts enables physicians to provide more equitable, timely, and effective care to patients across all skin types and ethnicities.
