Restrictive lung diseases are a group of pulmonology pathologies diagnosed via spirometry, demonstrating a “restrictive” pattern. This pattern of pathology demonstrates reduced lung distensibility, a key barrier to lung expansion, and thereby leads to reduced total lung capacity (TLC) and an increased ratio of expiratory to inspiratory capability on spirometry. Restrictive lung syndromes can be caused by intrinsic, or pulmonary, causes, which involve the lung parenchyma itself, while extrinsic restrictive lung diseases originate from neuromuscular disorders, obesity, and other extraparenchymal disorders. Developing a standardized approach to the evaluation of lung disease that involves history, physical examination, the use of pulmonary function testing, and imaging (when appropriate) is extremely useful in diagnosing restrictive lung disease.
